Why we're building this
Most chronic patients deteriorate slowly. The warning signs are visible days before the crisis — but only to the people who see the patient every day. Their family. Their carer. Not their GP, who sees them every few weeks.
We've watched that gap up close. A relative who stopped eating and didn't mention it. A grandmother who missed her evening medication twice and didn't think it was worth a call. The crisis that everyone could see coming, except the person who could have prevented it.
CommunityCare exists to close that gap. The patient stays where they're comfortable. The family logs the daily basics in thirty seconds a day. The GP gets the picture between visits that no one has had until now.
What we believe
- The hardest care is the invisible care. Family caregivers do most of it, and most of it is never written down.
- Software should reduce visits, not replace them. When you see your GP, it should be because something has actually changed.
- Coordination is not diagnosis. We give the picture. The clinician decides what to do with it.
- Privacy is non-negotiable. Patient data belongs to the patient, not us.

South Africa and the Philippines, then the UK. Three markets where chronic-care appointment volume is high, family care is the default, and the GP's time is precious.
