Early access — free until launch

Care coordination for general practice.

The patient's family or carer logs the daily basics. You get the picture you've been missing — without seeing them more often.

See how it works

Where we fit

Not a PMS. Not RPM. Care coordination — the layer in between.

Your practice management software runs your clinic. Remote monitoring devices read sensors. CommunityCare connects the patient's family, carers, and you — so what happens between appointments isn't invisible.

What it does

One circle. Three jobs. Done in seconds.

For the caregiver

One screen. Today's tasks. Tap, log, done. Less than a minute a day. Plain English — no jargon, no medical training required.

For the family

Everyone in the circle sees the same picture. Up to five family members per patient, free. No more group chats. No more phone tag.

For the GP

You only see what matters. Smart alerts when trend lines change — declining appetite, missed meds, falling mobility. Not a notification storm. Just the signal.

Inside the app

One product. Three surfaces. Each one calm.

Concept designs. Built for thirty-second use on the phone in the kitchen, and for the GP's screen in the consulting room.

How it works

Three steps. No training session required.

01

Invite the main caregiver

You send one invite per patient. Family member, paid carer, anyone. They join in under a minute.

02

Daily basics get logged

Meds, meals, vital signs, mood. Thirty seconds a day. The whole family circle sees it instantly.

03

You get only the signal

When the trend lines actually change, you get one alert. Pattern detected. Days earlier than you'd otherwise know.

FAQ

What GPs ask first.

Is CommunityCare a medical device or diagnostic tool?
No. CommunityCare is a care coordination platform. We do not diagnose, treat, or replace clinical judgment. We give you visibility between appointments and a clear record of why you acted, or didn't.
Where is patient data stored, and how is it protected?
Patient data is encrypted in transit and at rest, with role-based access and audit logging. Built to UK GDPR, POPIA, and Philippine Data Privacy Act standards. Patient data stays in your control. Patients and families can request export or deletion by emailing info@community-care.app.
Do the family members need to pay?
No. The main caregiver and up to five family members per patient are free. Optional extras — additional family seats, advanced learning content, extra storage — are paid by the family, not the GP.
How do you compare to remote patient monitoring (RPM) tools?
RPM connects devices. CommunityCare connects people. We give the GP a structured picture of the daily basics — meds, meals, mood, mobility — that come from family and carers, not sensors. Use both if you have RPM in place.
Can families really use it, or will I be doing tech support?
It's designed for a kitchen counter, not a clinic. One screen, one action, plain English. Most caregivers log their first observation in under thirty seconds with no training.
When can I get started?
Early access is open now. Book a 15-minute demo and we'll add you to the next cohort. Pricing is locked at the early-access rate for life.
